This job is responsible for the coordination, triage, registration, and communication of Telemedicine consult requests for the Health System. This job uses problem-solving and customer service skills to determine the best course of action for the patient by working closely with the practitioner to ensure effective and efficient treatment of every patient.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education
Required - Licensed Practical Nursing diploma
Work Experience
Required - None
Preferred - 3 years hospital based experience in the delivery of patient care as an LPN
Experience in Call Center, Emergency Department, Triage, or Telehealth setting
Certifications
Required - Current state licensure from an accredited School of Practical Nursing
Current Basic Life Support (BLS) certification from the American Heart Association

Job Duties
- Effectively uses the Nursing Process in the delivery of patient care through data gathering, planning, and evaluation to ensure the patient and provider are prepared for the Telemedicine visit.
- Completes appropriate documentation for all Telemedicine encounters, including registration if necessary.
- Coordinates and maintains data registry for each Telemedicine program.
- Provides excellent patient service through effective communication both over the phone and through video.
- Maintains proficiency in computer systems and databases for the purpose of excellent patient care, accurate data collection, monitoring, and evaluation.
- Maintains professional and technical knowledge and required licensure and certifications through continuous education and professional development and accepts new assignments and performs additional duties as requested in a professional manner.
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style. JOB IS FROM: partimejobs.netVIEW

Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
