The incumbent is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The NN assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). The NN has a particular focus on improving transitions in care and adherence to care. Additionally, the NN functions as a clinical advocate and educator for patients.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).

2. Obtain certification in Basic Life Support within 30 days of hire date.

EXPERIENCE:

1. Three (3) years of nursing experience.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelors of Science in Nursing Degree (BSN).

EXPERIENCE:

1. Care coordination experience is preferred.

2. Outpatient/clinic experience is preferred.

1. Assess and facilitate coordination of multidisciplinary care across the care continuum. Provide communication, advocacy, and education in a culturally competent manner.

2. Serves as the Pediatric Supportive Care team lead in providing supportive care to pediatric oncology patients/families during outpatient and infusion center visits. 

3. Serves as the liaison between Supportive Care and Ped hem/Oncology

4. Will serve as back-up for clinician providing supportive care to perinatal and inpatient pediatric families facing chronic, complex, life limiting and/or life-threatening conditions in conjunction with the Medical Director of the Pediatric Palliative Care Consultation Service.

5. Participates in interdisciplinary palliative care team.

6. Ability to clinically triage new patient referrals efficiently, to ensure timely access to care.

7. Collaborate with multidisciplinary care team and expedite sequence of patient's workup, active treatment, survivorship, and end of life care.

8. Provide communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients.

9. Targeted symptom assessment and management for identified patient population.

10. Assess health literacy and educational needs of patient. Provide clinical education about diagnosis, treatment, side effects, and post- treatment care.

11. Assess and document patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery.

12. Bridge gaps in care and assesses/troubleshoots care transitions and barriers to care.

13. Document all patient encounters in EPIC.

14. Possess leadership skills including staff education, preceptorship, patient education, and participate in process improvement initiatives.

15. Provides direction/guidance to Patient Navigator role.

16. Participates in all service line team meetings.
