Job Description
Provides direct and indirect services to patients and families. The core functions of a Resource Specialist include: identifying community resources, organizing these resources for the department, and linking patients and families to community agencies and services.
Onsite Part-time / Grant Funded
Essential Job Functions:
- Identifies community agencies and resources pertinent to respective medical service. Develops resources focused on the social influencers of health such as housing, food, transportation, and employment.
- Works directly with patient families to access resources for non-medical transition needs related to community supports, independent living skills, and future planning.
- Coordinates dissemination of resource information to Family Services staff and Medical Teams via in-services and material distribution.
- Develops relationships with community/government agencies to foster improved referral processes and resources for patient families.
- Maintains relationships with parent advocacy groups and organizations for specialized health and resource needs relating to patient families. JOB IS FROM: partimejobs.netVIEW
- Consults with families, social workers, hospital staff and the community regarding resource needs for patient families.
- Maintains up-to-date files on resources, their availability, appropriateness, special features, fees and eligibility requirements.
- Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.
- Other job functions as assigned.
Knowledge, Skills and Abilities:
- Bachelor’s degree in social work, vocational rehabilitation, rehabilitation & disability services, or human services preferred.
- One year of experience working with families of children with special health care needs.
- Comprehensive knowledge of resources and experience in working with them.
- Ability to think proactively, creatively, and take initiative appropriately. Demonstrates excellent communication and interpersonal skills. Demonstrates ability to work as a part of multiple teams. Spanish language abilities and skills strongly preferred.
- Stamina and energy to maintain direct patient contact on a regular basis with seriously ill persons and to coordinate multiple demands made on time.

$49,920.00-$81,619.20 SalaryAt Lurie Children’s, we are committed to competitive and fair compensation aligned with market rates and internal equity, reflecting individual contributions, experience, and expertise. The pay range for this job indicates minimum and maximum targets for the position. Ranges are regularly reviewed to stay aligned with market conditions.
