SUMMARY
With guidance from the Assistant Director of Patient Services, the RPM Supervisor is responsible for ensuring optimal daily operations of Remote Patient Monitoring Program and integration with clinical patient care. The Supervisor has direct oversight for 5-8 Telemed Nurses, and 2 Case Managers. The program is growing, and the Supervisor will play a key role in program development, streamlining processes, & assisting the team to embrace new technologies.
Key Responsibilities:
• Manages daily operation of RPM team
• Motivates, trains, supervises, evaluates and addresses disciplinary issues to ensure maximum utilization of individual and team capabilities.
• Review and develop RPM policies, procedures and practices to continually enhance efficiency and operations.
• Provides supervision for the RPM RN’s, Desk Case Managers, and Technicians

LOCATION: On-site in Rochester, NY
DEPARTMENT: Remote Patient Monitoring
SCHEDULE: M-F Days
ATTRIBUTES:
Bachelor’s Degree in Nursing required
Current New York State Registered Nurse license
Home Health field experience preferred
Clinical Experience with high risk patients preferred
Prior home care management/ supervisory experience or clinical expertise is preferred.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

PAY RANGE:
$75,712.00 - $100,880.00CITY:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
