This job plans, organizes and implements utilization management programs based upon individualized patient biopsychosocial assessments/reassessments and through the knowledge and use of the company and community based non-acute services to coordinate and implement complex discharge plans. Provides information and referral to governmental and community resources. Collaborates and communicates (verbally and through maintenance of the medical record) with the multidisciplinary team to (re)evaluate and respond to patient’s biopsychosocial and appropriate spiritual and cultural needs. Demonstrates actions consistent with the company's guiding principles as duties are performed on a daily basis. When needed will assist with daily clinical review of patient’s medical records for utilization review purposes. Works independently with minimal supervision under the direction of Registered Nurse Case Managers. Communicates the clinical review either verbally or in writing to insurance companies, Registered Nurse Case Manager or other members of the health care team. Performs duties under general supervision and following complex procedures and guidelines.Schedule: 2 days per week, as needed

Education
Required - Associates degree
Work Experience
Required - 1 year experience in Health care.
Certifications
Required - Registered Nurse in the state of Louisiana
Preferred - ACLS, PALS, BLS
Knowledge Skills and Abilities (KSAs)
- Must have computer skills and dexterity required for data entry and retrieval of information.
- Effective verbal and written communication skills and the ability to present information clearly and professionally.
- Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
- Strong interpersonal skills.
- Possess the ability to respond to high volume, fast paced clinical and office setting.
- Dynamic knowledge of managed care and public programs such as those funded by Medicare (especially Medicare risk products), Medicaid, and other payers 3rd party.
- Maintain a professional, neat work environment, manages stress in order to maintain a professional demeanor and perform multiple tasks simultaneously.
- Ability to promotes a positive image of the organization while maintaining strict confidentiality.
Job Duties
- Under the direction of the Case Manger Director perform Discharge Planning functions for the Case Management Department.
- Responsible for arranging referrals for post discharge needs of patient and or family choice.
- Communicates with physicians, nurses, therapies, case managers and third party payer regarding the discharge plan.
- Under the direction of a RN Case Manger performs utilization review functions for the Case Management Department.
- Demonstrates participation in professional growth and development, maintaining required knowledge and skills needed to perform the duties of the position.
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
Normal routine involves no exposure to blood, body fluid or tissue, but exposure or potential for exposure may occur.
The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
