Overview
Key responsibilities include but are not limited to:
1. Navigates patient care from initial referral to the specialty until consultation with care team.
- Coordinates care for new patients.
- Works with patients and families to prioritize patient needs and goals for initial appointments.
- Anticipates future needs of patient and educates and/or refers to appropriate sources.
- Assesses patient clinical history and, if known, diagnosis, to create an initial plan for provider appointments and, if appropriate, further diagnostic tests.
- Collaborates with schedulers to schedule initial appointment(s) and assists scheduling team to resolve any barriers to carry out appointment plan.
- Provides patient/family education regarding additional testing and work up which is needed.
- Develops and engages patient and family in plan of care.
- Documents and updates EHR as initial appointment care plan is developed.
- Excellent written and verbal communication skills.
- Knowledge of specialty diagnoses.
- Strong nursing assessment skills.
- Collaborates with records team to obtain notes, imaging, results, etc… from outside the health system.
2. Serves as resource to referring offices regarding GI specialty.
- Maintains good communication and relationship with team and referring office
- Communicates providers to gain additional clinical history and/or inform of the initial appointment plan. Also communicates recommendations based on provider review of patient.
- Strong relationship building skills.
3. Demonstrates ability to deliver care/service adjusting approaches to reflect developmental level and cultural differences of population serviced.
- Understands utilization of interpreter services
- Works with social workers to identify social barriers to care
- Sensitivity to diverse populations
- Covers for other sub-specialties within the navigation team.
What you will need:
- Associate’s (ADN) or Bachelor’s Degree in Nursing (BSN) and current Minnesota RN license
- BLS certification
- 5+ years of hospital or clinic experience, or 2+ years of specialty/critical care experience with specialty certification
- Experience with electronic health record systems
Location: Remote
Hours: 1.0 FTE

Compensation:
