Develops, implements, and coordinates the Bereavement Care Program focused on the family and patient/resident experience. Develops individualized plan of care for families to cope with physical, emotional, and spiritual needs.Job Description
Provide guidance and plan of care for multidisciplinary caregivers to maintain consistent approach and support. Establishes working relationships and collaborates with all local support groups and facilitates the internal Bereavement support group. Coordinates the program services activities.

Coordinates the logistical implementation of the program, and adheres to program actions and deliverables based on program grants when relevant. Manages multiple concurrent projects and/or priorities. Establishes project plans that identify, communicate, and track tasks and deliverables to meet goals/objectives. Evaluates key metrics related to project goals/objectives. Coordinates communication and related committee meetings with internal and external partners. Utilizes interdependence in the workplace and on work projects within the department. Assesses and monitors potential or actual problems and implements resolutions needed for successful outcome. Respects and maintains confidentiality.
Work hours vary according to department and client needs. May be required to work evenings and/or weekends. Manages social media sites and creates content for the website. Tracks inventory and completes program supply ordering.
May collaborate with Physician Palliative Care provider for initial consult and Spiritual Care to provide grief and bereavement support services to patients/residents and families affected by trauma or sudden serious illness and death.
Qualifications
Bachelors degree required in related work field (i.e. Child Development, Family Science, healthcare); Masters degree preferred.
Previous hospice, end of life, or grief counseling training preferred.
