Job Summary:
CareLinx by Sharecare is seeking a Clinical Program Manager to oversee government healthcare programs, including the GUIDE Model, and provide operational leadership across multiple initiatives. This role will also be responsible for ensuring compliance and quality oversight for in-home caregiver visits. The Clinical Program Manager will play a key role in integrating program operations, ensuring regulatory compliance, and driving quality improvements in home-based care.
This position serves as a key clinical resource for identifying risks in the home and supporting health insurers with early intervention strategies. The ideal candidate will be comfortable working in a fast-paced, technology-forward environment, collaborating with interdisciplinary teams, and traveling locally for in-home care oversight.
***Candidate must be able to travel locally in the Atlanta area (up to 40%) for in-home care visits and field assessments.
Essential Job Functions:
- Program Oversight & Operations:
- Lead the clinical management of government programs such as the GUIDE Model, ensuring adherence to federal and state regulations.
- Provide operational oversight across multiple home- and community-based programs to ensure seamless service delivery and quality care.
- Collaborate with Sharecare’s legal and compliance teams to ensure programs meet all regulatory and contractual requirements.
- Act as a liaison between CareLinx, payers, and government agencies to support program implementation and ongoing operations.
- In-Home Caregiver Supervision & Quality Monitoring:
- Oversee in-home caregivers, ensuring that services align with member care plans and program guidelines.
- Monitor and analyze real-time data from caregiver visits to identify gaps in care, trends, and risks.
- Conduct periodic field visits to assess quality of care, adherence to care plans, and patient outcomes.
- Develop and implement quality improvement initiatives based on data insights.
- Clinical Risk Identification & Early Intervention:
- Serve as the clinical lead for risk identification in home-based care settings.
- Use data and field assessments to identify high-risk patients and collaborate with health plans on early intervention strategies.
- Work closely with payer partners to align value-based care strategies with in-home care delivery.
- Process Improvement & Compliance:
- Implement best practices for operational efficiency, caregiver engagement, and clinical workflows.
- Ensure all caregiver documentation and reporting meet CMS, state, and program-specific guidelines.
- Lead ongoing education and training for caregivers on program requirements and best practices.
- Monitor compliance metrics and recommend quality improvement initiatives to leadership.
Qualifications:
- Bachelor’s degree in Nursing required.
- Active Registered Nurse License in the state of Georgia required. Multistate privileges preferred or ability to obtain.
- Minimum 3 years of clinical experience, with at least 2 years in case management, care coordination, or program oversight.
- Experience working with government healthcare programs (e.g., CMS models, Medicaid, Medicare Advantage, GUIDE Model, etc.).
- At least 2 years of supervisory experience overseeing non-clinical care teams or caregivers.
- Strong knowledge of value-based care models, home health operations, and compliance requirements.
- Experience with data analysis, care quality metrics, and risk identification.
- Proficiency in Microsoft Office Suite and EMR platforms.
- Ability to travel locally (up to 40%) for in-home care visits and field assessments.
- Strong organizational, communication, and leadership skills to drive operational excellence.
- This role will be home-office based and must meet HIPAA security requirements and minimum internet connectivity standards.
